Academic advising proxy

An Academic Advising proxy form is issued when a student is unable to act on her or his own behalf. Reasons may include hospitalization, illness, mental health issues, and language barriers. By completing and signing an Academic Advising proxy form, a student is giving permission for another individual to act on their behalf as it pertains to their advising record in the Academic Advising Centre. The Academic Advising Centre recommends students provide proxy for the minimum amount of time necessary. Consent may be given for a maximum of 12 months and then renewed as needed. Students should indicate which areas of their Academic Advising record they are releasing for the proxy.

Consent to participate in an advising consultation

A signed consent to participate in advising consultation form must be presented by the student at the time they are booking an appointment. Once a student has started courses at the University of Victoria, this form is needed when the students would like to have another person (e.g. parent, guardian, friend) attend a meeting with an academic adviser. By completing and signing this form, a student is agreeing to share personal information with the person identified on the form.

Program declaration or change

Submitting a declaration lets UVic know of your intention to complete a specific academic program and will help you track your program progress through a personalized CAPP (Curriculum, Advising and Program Planning) report that will be prepared for you. Changes to your declared program may be made online once per term - if further changes are required please visit the Academic Advising Centre.

Keep in mind:

  • Before submitting a program request, students must have a minimum 12 units of completed course work.
  • To declare an Honours program, you must first obtain approval from the Honours Adviser within the appropriate Academic Unit. The Academic Unit Adviser (AUA) will notify the Advising Centre of the your admission to the Honours program.

Request for academic concession

Circumstances may mean a student will want to request an academic concession during your studies. An AAC adviser can provide advice and help so students understand their options. If a student experiences illness, an accident, or family affliction that prevents the student from completing his or her course(s), the student will need to submit a request for academic concession (RAC), as well as accompanying documentation, as soon as possible to Undergraduate Records in the University Centre. Approval of the request may give the student additional time to complete course work, or allow for a course drop without academic penalty.

Request for letter of permission

For students who want to take a course at another college or university, they will need to submit a Request for a Letter of Permission (LoP). Caution: students using transfer credit towards a UVic degree will need to consider degree requirements. It is students’ responsibility to ensure all degree requirements are met. A LoP will clarify how credit will transfer to UVic (no credit, level credit, course credit), not how it will apply to your degree.
Please note: You must submit this request form before you commence studies at the visiting institution.

Request for pre-requisite waiver

Students who believe they have sufficient background to be exempted from a specific pre-requisite for a course may request a pre-requisite waiver by submitting the waiver request form to the Department or School offering the course. It is the student’s responsibility to provide any relevant documentation to support their request – see instructions on page 2 of the form for details. Students should contact the Department/School main office to find out where to electronically submit their waiver form

Request for surplus credit allocation

Prior to graduation, students may realize they have completed more courses that are required to meet their degree requirements. These extra courses might be applicable to a future diploma or bachelor’s degree. An adviser will review the student’s record and determine if he or she has any surplus courses that qualify to hold out from the degree. Students must complete this process prior to graduation, otherwise unused credit cannot be applied to future qualifications.

Request for course overload

Students submit a Request for Course overload if they are wanting to take more than the maximum. 7.5 units during a single term is considered an ordinary course load. However, students are permitted to register in up to 9.0 units of courses in each of the First (Fall) Term, Second (Spring) Term, and Summer Session (May-August). If you wish to request registration in courses additional to the limits identified above, you must complete and submit this form including the explanation of your request. Submitting the form does not guarantee approval.
