Hazard reporting

Any campus health and safety concern or hazard should be reported as soon as possible to avoid potential accidents or injuries.

For medical or emergency assistance, first call 911 and then call Campus Security at 250-721-7599.

Faculty, staff & students

Report health and safety concerns or hazards to your supervisor and document using the Department Incident and Investigation Report form. If you are unable or uncomfortable reporting to your direct supervisor, please report to another management representative from your department or to your union representative, local safety committee or OHSE

Managers and supervisors are responsible to address reported health and safety concerns or complaints in a timely manner. 

If a reported safety issue or concern remains unresolved, the individual should contact their joint local safety committee for assistance. This will provide an opportunity for management and employee representatives, along with OHSE, to review the issue and action next steps towards a resolution.

Visitors & contractors

Visitors should report any campus health and safety concern or hazardous condition to Campus Security at 250-721-7599. Contractors should also report any concerns or hazardous conditions to the assigned Project Manager in Facilities Management or departmental contact person.

Supports and resources

Some safety issues or concerns may involve notification to a specific department to address the issue, including:

Facilities Management

  • Any non-emergency issues involving plumbing, electrical, heating and ventilation, building infrastructure or maintenance (e.g. leaking pipes, lighting, torn carpeting, loose hand-rails, etc.).
  • Any concerns observed on campus property including slip/trip hazards on sidewalks, building entrances, or grounds.

Campus Security

  • Any concerns about parking areas including lighting, equipment, or slip/trip hazards.
  • Any concerns about suspicious behaviour, theft or personal safety (including potential aggression, violence or threatening behaviours).
