Students, staff and faculty must declare their COVID-19 vaccination status by Sept. 26 (midnight).

If you declare that you are not fully vaccinated or choose not to disclose your vaccination status, you must participate in weekly rapid testing.

UVic has partnered with Thrive Health to provide easy access to self-declaration forms and follow-up testing. Your declaration is confidential and will be securely stored.

Log in with your Netlink ID.

How to self-declare

  1. Log in to My Thrive Health. You will see a task card on your home screen to complete your required self-declaration.
  2. Acknowledge the mask requirements and indicate whether you require a medical exemption so we can follow up.
  3. Confirm whether you have received two doses of the COVID-19 vaccine.
  4. You will receive confirmation of your submission.

Verifying your self-declaration

You may be randomly selected to provide proof of vaccination through Thrive Health. All information provided will be kept confidential.
