Academic forms

These forms have been converted to fillable PDF’s to make them easily accessible and user-friendly.  To use these forms, see the instructions below:

  1. Once you open document, save as a new PDF to your computer
  2. Fill out the form accordingly and be sure to save periodically
  3. Create a Digital ID to authenticate your signature (for more information, please click here)
  4. Save the PDF and attach it to your e-mail to his@uvic.ca

Biology Waiver Request Form (Request to Change CAPP Report)

Students with Grade 12 Biology or Physiology and Anatomy 12 must complete a Biology Waiver Request form and must take 1.5 units of additional elective (which can be one of these courses). Students without credit for Biology 12 or Physiology and Anatomy 12 must take one these courses.

You will be notified by email, when completion of Grade 12 Biology or Physiology and Anatomy 12 has been verified and your CAPP report has been updated.

Pre-requisite Waiver Form

The pre-requisite waiver form is used to assess progress in the HINF program. This form is only for HINF students and granted under extenuating circumstances. 

If you are a student in another program and are interested in taking a HINF course but have questions about pre requisites/eligibility, please reach out to his@uvic.ca.

If approval is granted to register in a course without the required pre-requisite(s) it is important to note that successful completion of the missing pre- requisite(s) in a future term is still required for degree completion.

Please attach any relevant supporting documentation, such as proof of registration at the other institution, transfer credit summaries, or unofficial transcript.

Letter of Permission (LOP)

Letter of Permission (LOP) to Undertake Studies at an Alternate Institution

Students wishing to complete courses at an alternate institution are required to complete a Letter of Permission (LOP) prior to enrollment.  You may use the BC Transfer Guide as a reference to see if it matches an equivalent course or rule at UVic.

Students are subject to the regulations of the University of Victoria governing academic standing and credit. Transfer credit is only awarded for courses completed successfully.  If credit for any of the course(s) listed above, or their equivalent(s), has been previously obtained, no further credit will be granted.  Grades are not transferred and will not affect your UVic GPA. Nonetheless, your grades from this institution will be used to calculate your UVic academic standing and may lead to be placed on probation or being required to withdraw.
