Video recording request form

Please note that due to the upcoming Congress of the Humanities and Social Sciences we are not booking video recordings from May 27, 2013 through June 10, 2013.
 

To ensure we can accommodate your request we require the submission of this request form a minimum of two weeks prior to the start of the event.

Please note: DVDs are labelled according to the information you provide with this form; be sure to enter the Name of Lecture and Presenter's name exactly as you would like them to appear on the finished DVDs.  We do not keep copies.

Cancellations must be received 48 hours prior to the video recording date or full charges will apply.

Please obtain written permission to video record the event from the presenter.
