Changes to Exam and Survey Scanning Services

At the request of faculty and after a full year of work by the test scoring faculty working group, a new exam and survey scanning service was launched in September 2014.  

Effective May 1, 2015 the Computer Help Desk and University Systems will only be accepting the new UVic bubble sheet forms which can be purchased from the University of Victoria Bookstore. The new forms available are the General Purpose Five Response Answer Sheet ( SKU 40031400001 ) and the General Purpose Ten Response Answer Sheet ( SKU 40031400002 ).

The old bubble sheet forms ( rev date  5/97 ) and the old survey forms can no longer be accepted after this date as the scanner has reached end of life.

Old forms received after May 1, 2015 will be returned to the department.

The implementation phase for this service is now complete.  The ongoing administration of the service will now be transferred to the LTC as of March 15, 2015.  If you have any questions after this date please email ceshelp@uvic.ca.
