Supervisor permission form

JCURA students wishing to have their poster/files/project considered for inclusion in the online JCURA Poster Collection housed in UVicSpace must have the approval of their supervisors. Completing this online form grants your permission for the student you supervised to submit his/her poster. Please read the following.

In consideration of the University of Victoria (“UVic”) agreeing to store my JCURA student’s poster/files/project and facilitate access to the submission by others, I, the JCURA supervisor:

  1. In consideration of the University of Victoria (“UVic”) agreeing to store my JCURA student’s poster/files/project and facilitate access to the submission by others, I, the JCURA supervisor, have checked that:
    1. Images that are protected by copyright have the required permission by their creator
    2. Quotations and paraphrases that are correctly cited
    3. I am comfortable with having the material available in UVicSpace
    4. A substantial amount of work has not been produced by another student, research group, lab, etc. If the submission contains third party material, this third-party material is clearly identified and acknowledged within the text or content of the submission, and the use of such material complies with the fair dealing provisions of the Copyright Act (Canada) or the student has obtained the unrestricted permission of the copyright owner to include the material in my submission and to grant UVic the rights required by this license.
  2. I also agree that my name and department are clearly listed on the poster/files/project destined for digital storage on UVicSpace.

Students are required to have their JCURA supervisors’ approval to have their poster/files/project considered for inclusion in the JCURA Poster Collection on UVicSpace. Completing this form does not guarantee the poster/files/project will be accepted. Students whose work has been accepted must agree to the Creative Commons license.
