UVic has partnered with Thrive Health to provide easy access to self-declaration forms and follow-up testing. Your declaration is confidential and will be securely stored.

How to self-declare

  1. Log in to My Thrive Health. You will see a task card on your home screen to complete your required self-declaration.
  2. Confirm whether you have received two doses of the COVID-19 vaccine.
  3. Upload a photo or PDF that provides proof of vaccination.
  4. You will receive confirmation of your submission.

Providing proof of vaccination

If you're updating or submitting your self-declaration you'll need to provide proof of vaccination. Please be ready to upload a photo or PDF document that provides proof of your vaccination status.

We will only review records that are randomly selected as part of our self-declaration audit.

All information provided is kept confidential.

Privacy & security

The personal information that you provide is being collected by Thrive Health Inc. on behalf of the University of Victoria under the authority of the University Act and section 26 (a), (c), and (e) of the Freedom of Information and Protection of Privacy Act (FIPPA)Your personal information will be used to manage the health and safety of individuals attending activities on campus during the COVID-19 pandemic. If you have any questions about the collection and use of your personal information, please contact the Privacy and Access Information Office.

All user data is stored on Thrive Health's secure web platform under the control of UVic and in accordance with FIPPA and UVic’s Protection of Privacy Policy and Information Security Standards.
