How to make a freedom of information request

A formal freedom of information request must be in writing, except in limited circumstances.

Official Freedom of Information forms are stored in Adobe Acrobat PDF format. To view PDF files, you will need the Adobe Acrobat Reader program. If you do not have this program installed, download a free copy of Adobe Acrobat Reader.

  • Or, make your request via letter or email
    • Provide your contact information
    • Provide details of the records/information you are seeking. Be as specific as possible.
    • Indicate whether or not you are requesting access to another person's personal information. If yes, attach that person's signed consent for disclosure to you, or proof of authority to act on that person's behalf.

If you are requesting your own personal information, it may be possible for you to receive it without making a formal freedom of information request. Please first direct your request to the office that holds your information.
