Name: student attending UVic / name on Food Card
Phone Number: contact number of payee in case of errors
Address Line 1: payee current address
Address Line 2: payee full name
City: payee current City
Province/State: payee current Province/State of residence
Postal/Zip: payee current Postal/Zip code
Country: payee current Country
Email: payee email address
Invoice/PO: supply the UVic student ID number. Starts with "V". All other Food Card types please use full number located on Food Card.
Please provide the date (MM/DD/YYYY) of the submitted payment and any additional comments in the comment section.
