Please complete the following information only if you are also submitting an application for graduate studies at UVic (see the “To Apply” tab in the UVic Academic Calendar for steps involved in submitting an application). The information below is supplemental and is for departmental use only and the completion of this form does not mean that you have completed your application for graduate studies. You must complete all the steps outlined at the link above, submit all of the required documentation, and pay your application fee to UVic in order to apply for graduate studies. We would also appreciate your filling in and submitting this form to the Neuroscience program, as it will greatly assist us in our admission decisions.
